CHADIS (Child Health & Development Interactive System) is an online screening tool used for early diagnosis of health, developmental, and behavioral issues. Your HPA pediatrician has selected screening tests based on your child's age and/or specific needs or concerns.
Using CHADIS is easy. Navigate to www.chadis.com and register as a new patient. You'll be asked for your invitation code. Please call 256-888-KIDS (5437) to obtain your code. You'll be instructed which questionaire to complete. Only one registration is required per family.
You'll be able to complete a questionaire on CHADIS two days prior to your child's scheduled well child appointment, beginning with their six-month check up and every check up thereafter. Your pediatrician will have access to the confidential results and interpretation. These results will be available for discussion during your visit.
